Host Site Final Reporting

To receive your final payment, please complete the evaluation below with the following forms attached.

• Attendance Form (You will need to attach this form to the Host Site Evaluation below.)
• Host Site Final Report: Compile a final report on the results of your Family R&D Program including the answers to the follow questions. (Some of this report will elaborate on your responses to the Host Site Evaluation below.) Please feel free to include any additional information or comments regarding your experience facilitating and hosting your Family R&D Program. (You will need to attach this form to the Host Site Evaluation below.)
  • Were you satisfied with your experience hosting a Family R&D Program? What were your successes? What challenges did you face?
  • Did the program meet your expectations? Did the program meet your personal or organizational goals?
  • In what ways did your Family R&D Program diverge from the Together or Unidos model?
  • Would you recommend this series (book list and theme(s)) to another group? Why or why not? What changes would you make?
  • What feedback did you receive from participating families?
  • What recommendations would you make for future facilitators?
  • What recommendations would you make for future host sites?
• Blog Entry: Please provide a short entry for the Together facilitator blog about your experience hosting a Family Reading & Discussion Program. (This can be taken directly from your Final Report.)
• Host Site Evaluation (requires Attendance Form and Final Report as attachments)

Please note that Host Site Coordinators are responsible for ALL the forms listed above, as well as the submission of the scholar co-facilitator's evaluation.
