Please note that Host Site Coordinators are responsible for ALL the forms listed below, including the distribution and return of participant evaluation forms, and the submission of the facilitator's evaluation. Final payment is contingent upon the receipt of all evaluation forms, as outlined in your Grant Agreement letter.
